CORE Peer Advocate
S.T.E.P. by S.T.E.P., Inc.Position Summary:
To provide peer support and rehabilitation services to persons who are HARP eligible in St. Lawrence County while modeling an articulation of personal healing and recovery.
- The essential functions of this position include, but are not limited to:
Developing individual service plans with individuals who are enrolled - Meet with eligible peers to provide peer support in completing the objectives of the ISP throughout the county (i.e. DSS, SSI, housing, employment opportunities, etc
- Enter progress notes in the electronic health record
- All other support activities as requested by the individual
- Be available to work weekend and evening shifts (flexible hours).
- Other duties as assigned by supervisor.
Qualification:
Must have excellent communication, computer, and organizational skills. Must be familiar with use of the internet and possess good research skills, as well as the ability to get along well with people. Must have an excellent working knowledge of entitlements available to the general population. Must have a valid drivers license and reliable transportation. Participate in ongoing training. Position requires travel within St. Lawrence County.
