We are actively seeking a highly motivated, innovative and experienced leader to join our team as the Community Health Outreach Worker. Compensation will be commensurate with experience.
Position Summary:
The Community Health Outreach Worker (CHOW) is a part-time position that assists with the outreach, education and counseling of a specified group of members; consistent with the philosophy, goals and objectives of Amida Care. Assists clients in adhering to their individual disease processes and care management plans. Incumbent may be a Plan member of Amida Care.
Responsibilities:
- Provide assigned group of members with general information pertaining to the Plan and healthcare access.
- Facilitate access to treatment programs.
- Provide travel assistance/escort for specific members to scheduled appointments.
- Conduct community canvas to locate additional resources and/or possible client locations.
- Foster trust in healthcare system and promote treatment adherence.
- Act as patient advocate in specific situations and/or discussions with care providers.
- Counsel designated clients on basic harm reduction information.
- Provide social support and enhancement of self-esteem to assigned clients.
- Weekly documentation of outreach efforts in Amida Care member management systems.
- Monthly New Member Orientation home visit outreach.

EDUCATION REQUIRED
- High School Diploma or equivalency
EXPERIENCES AND/OR SKILLS REQUIRED
- Completion of peer training preferred.
- Demonstrated experience/ participation in community activities.
- Demonstrated judgment of safety, boundaries and confidentiality issues.
- Demonstrated competency in using Microsoft Outlook, Word, Excel, and ability to learn to use proprietary applications.
- NYS Peer Certified preferred
- PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to communicate. The employee occasionally is required to sit at a desk and sometimes use a computer. The employee is constantly required to stand; walk; ride the bus/subway to all 5 boroughs of New York City.
- The employee commutes, approximately 75% of the time, to meetings, trainings, and home visits as scheduled throughout the year. The employee spends approximately 25% of the time writing and/or keyboarding.
- WORK ENVIRONMENT: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee works mostly outside and sometimes in a variety of weather conditions. The noise level in the work environment is usually moderate and sometimes noisy.
