Per Diem Family Peer Support Worker
CAPTAIN Community Human ServicesA successful Candidate for this position must have lived experience with a child/ youth who have developmental disabilities, social/ emotional challenges, addiction, and/ or behavioral health challenges, in their school, home and community. This position requires flexibility, travel, and providing services to families and their children in their home. Family Peer Support Services are provided by an individual who holds a Family Peer Advocate credential. Service Counties include; Saratoga, Warren, Washington, Fulton, Montgomery, Schoharie, Schenectady, Rensselaer, and Albany.
Family Peer Support Workers collaborate with the Care Manager and family to design appropriate goals to support the parent/caregivers in advocating for their child to live successfully in their homes, school, and within their communities. These services include developing parenting skills, empowering, building support services and connecting with groups within their community.
