Pay Rate: $41,600-$46,600 annually, commensurate with qualifications
Schedule: This is a full-time, non-exempt position. There are 2 positions available for this role: Position 1 schedule: Tuesday-Saturday, 10am-6pm; position 2 schedule: Sunday-Thursday, 8 am-4pm
Most work at CHASI happens face to face. Occasional remote days may be incorporated into the position’s schedule if required for administrative duties (by supervisor approval).

Position Description:

CHASI’s Next Step Resource and Recovery Center is a 24/7 program that provides opportunities for people in all stages and forms of recovery to deliver and attend individual and group support activities, initiate and conduct outreach, and events celebrating recovery and reducing stigma associated with substance use. The Care Coordinator is a key member of Community Health Action of Staten Island (CHASI)’s Connections to Care program at Next Step. The Connections to Care program provides comprehensive assessment, linkage and navigation services to support the long-term substance use disorder recovery and overall wellbeing of individuals returning to the community from criminal justice settings. Care Coordinators work very closely with other members of the Next Step team providing substance use support services such as the Peer Mentors, Counselors, Housing Advocate, and Workforce Development Specialist to help members of this underserved population reintegrate and lead fulfilling lives.

Essential Functions: 

  • Conduct screening and intake of eligible clients;
  • Complete comprehensive biopsychosocial and vocational assessment of enrolled clients;
  • Develop service plans with actionable and realistic objectives with clients;
  • Gather the necessary consents, program agreements, and service plan signatures;
  • Provide referrals, initial appointment scheduling, accompaniments, and confirm linkages;
  • Coordinate care and treatment needs with service providers outlined in the service plan; including key members of the Next Step Resource and Recovery staff team;
  • Participate in conferences as needed with other service providers;
  • Promote the program through outreach and program presentations;
  • Outreach to the person’s support system/ family to gather information and support in service planning when appropriate;
  • Participate in quality Assurance activities and quality Improvement projects;
  • Complete all program documentation in a timely manner;
  • Align all activities with CHASI’s mission, core values, and culture

Requirements

 Qualifications:

  • At least 2 years of experience working with justice-involved populations with additional experience working with people having a history of homelessness, mental health challenges, substance use
  • Bachelor of Arts in Social Work or related field preferred; High School or equivalency required
  • Bilingual (English/Spanish) highly preferred
  • Knowledge and understanding of care management principles, documentation, referral processes, and follow up
  • Possess an understanding of community-level work and the importance of collaborating and coordinating with other organizations
  • Ability to identify issues, obstacles, and opportunities and implement effective solutions; willing to be creative and be open to new ideas
  • Effective communication skills, including the ability to follow instructions, listen, and communicate effectively in writing and verbally; able to remain professional and calm in challenging situations
